Fast Track Care

If a person’s condition is both rapidly deteriorating and may be entering a dying phase, Fast Track Care may be required for immediate provision of NHS Continuing Healthcare. NHS Continuing Healthcare is a care package, funded and arranged by the NHS. Continuing Health review this process regularly. Further information about Fast Track Care is available here.

NHS Continuing Healthcare can be provided in a variety of settings outside hospital, such as in your own home or in a care home.

Information and advice is accessible in the form of free and comprehensive written guidance, and individuals are also able to access up to 90 minutes of free advice with a trained NHS continuing healthcare adviser 0345 548 0300.

Medicines

The following is a list of pharmacies that stock commonly used palliative medicines and pharmaceutical drugs. Other pharmacies may hold stocks of these medicines, but it may be wise to contact any pharmacy not on the list first to check stock availability and avoid a potentially wasted journey:

Yellow weather warnings in our area

The Met Office has issued yellow weather warnings for ice and snow in our area. This is expected to affect routes to and from our hospital sites. 

If you are attending for an appointment, please plan ahead. Check road closures, leave in plenty of time, and drive safely.
