Long Service Awards: Application form

Our long service annual awards proudly recognise staff who have completed 10, 25 and 40 years’ consecutive service within the NHS.  Staff who have transferred in or been part of a TUPE arrangement are also eligible.

  • Employees celebrating 10 years receive a certificate and pin badge.  Please note that this is a new award and is only relevant to those who achieved their 10 years service in the financial year 2020/21 onwards.
  • Employees celebrating 25 and 40 years' are invited, along with a guest, to an evening dinner and drinks reception, where they are presented with a certificate, a Yorkshire traditional hamper and a long service pin badge.

Most, if not all staff, are captured automatically and will be contacted if they are eligible - but we know occasionally the system is less than perfect, so we also ask staff to complete the online form below to ensure no-one is missed.

Update for visitors
