Transition coordination service

Welcome to the transition coordination service

Transition can be defined as a purposeful, planned process that addresses the medical, psychosocial and educational/vocational needs of Children and Young People (CYP) with chronic physical and medical conditions as they move from child-centred to adult-oriented health care systems.

Transitioning to adult services can be a confusing and overwhelming period in a child’s journey. It can be difficult to know when to start the process, and navigating all the systems involved to get the right care can be challenging.

The Transition coordination service was established at York Hospital in 2021 to support young people in their transition from the child-centred into the adult-orientated healthcare services - this process is widely known as either transition or preparation for adulthood.

The service is nurse led and takes a holistic and individualised approach, where children and young people are supported in a planned and purposeful way which addresses their medical, psychosocial, and educational/vocational needs to best prepare them for transition into adulthood and adult health services. This is so that they can achieve maximum potential and, where possible, are given the skills and knowledge to feel confident in taking control of their own health and accessing healthcare provision so they can make informed choices and decisions about this, both now and in the future.

The transition coordinator is Richard Bentley, he can be contacted on yorktransition@york.nhs.uk. Referrals into the transition service are made through your clinical specialist or other professional.

Update for visitors

Visiting restrictions are in place at our hospitals to stop the spread of norovirus. These restrictions will be in place until further notice.

The following wards are closed to visitors:

  • All wards at York Hospital
  • Coronary Care and Ash Ward at Scarborough Hospital

There are four exceptions only to the restrictions. These are:

  • For patients who are receiving end of life care
  • Paediatric patients (babies and children)
  • Maternity care
  • The family carers of those with dementia

We understand these restrictions can be difficult when a loved one is unwell. The restrictions will be reviewed daily and lifted as soon as it is safe. Thank you for your understanding as we try to reduce the spread of norovirus in our hospitals.
